Why Vaccination Matters in Pakistan
Vaccines protect children from diseases that can cause disability or death, and many of these illnesses are still a threat in Pakistan. Over the years the EPI has added new vaccines, including pneumococcal (2012), injectable polio (2015), rotavirus (2017) and typhoid conjugate vaccine (2019 in Sindh, nationwide by 2022). In 2021 the measles vaccine was replaced by the measles-rubella (MR) vaccine, which also protects against rubella and congenital rubella syndrome.
Polio remains a particular priority. Every OPV and IPV dose counts, both at the health centre and during campaign days.
An unvaccinated child is at risk, and can also pass infections to babies who are too young to be vaccinated. That is why every dose on time matters.
Typhoid in Pakistan and the TCV Vaccine
In November 2016 an outbreak of extensively drug-resistant (XDR) typhoid began in Hyderabad, Sindh. XDR typhoid resists most first-line antibiotics, so prevention matters far more than treatment. By late 2019, more than 11,000 cases had been reported in Sindh.
On 15 November 2019 Pakistan became the first country in the world to introduce typhoid conjugate vaccine (TCV) into its routine immunization programme, starting with a campaign in Sindh that reached more than 9.4 million children aged 9 months to 15 years. TCV was extended to Punjab and Islamabad in 2021 and to the whole country in 2022. It is now given at 9 months, along with MR-1 and IPV-2.
What If a Vaccine Is Late?
Do not panic and do not restart the schedule. Give the missed vaccine as soon as possible and continue from where your child left off. Doses already given do not need to be repeated. Rotavirus vaccine has an upper age limit, so do not delay it. Ask your doctor or EPI centre about catch-up vaccination.
Vaccination Side Effects in Children
Vaccines are safe and carefully tested, but like any medicine they can cause mild, short-lived reactions. These usually settle within 1 to 2 days.
| Vaccine | Common reactions |
|---|---|
| BCG | Small sore at the site that heals into a scar; sometimes a swollen gland in the armpit |
| Hepatitis B, Pentavalent, PCV, IPV, TCV | Mild fever, pain, redness or swelling at the injection site, irritability, sleepiness |
| Rotavirus (oral) | Mild diarrhoea, vomiting or irritability for a day or two |
| OPV (drops) | Usually none |
| MR (measles-rubella) | Fever and a mild rash about 1 to 2 weeks after the dose |
What You Can Do at Home
- Feed your baby often and keep them comfortably dressed.
- Place a clean, cool, damp cloth on a sore injection site.
- Ask your doctor before giving any fever medicine, because the dose depends on your child's weight.
When to See a Doctor
- High fever that does not settle, or fever lasting more than 2 days
- Crying non-stop for more than 3 hours
- Repeated vomiting, or blood in the stool after the rotavirus vaccine
- Seizure or unusual drowsiness
- Swollen face, widespread rash or difficulty breathing (go to the nearest hospital immediately)
Where to Get EPI Vaccines
Visit your nearest government hospital, Basic Health Unit (BHU), Rural Health Centre or EPI centre, or ask your Lady Health Worker. Always get each dose recorded on your child's official EPI card and carry it to every visit.
