OGTT in Pregnancy: Test Procedure & Results Explained

What Is OGTT?

The Oral Glucose Tolerance Test (OGTT) is a laboratory test used to assess how well the body uses and controls glucose (sugar). After drinking a specific amount of glucose, blood sugar levels are measured at different time intervals.

OGTT is commonly used to diagnose:

  • Prediabetes
  • Type 2 Diabetes
  • Gestational Diabetes Mellitus (GDM)

Because this test evaluates the body's response after a glucose load, it can detect abnormalities in sugar metabolism that may not be picked up by a fasting blood sugar test alone.

OGTT (Oral Glucose Tolerance Test) Procedure

OGTT is a test performed to check how effectively the body is using glucose (sugar). It plays an important role in diagnosing diabetes and gestational diabetes.

75 g OGTT (generally used to diagnose diabetes or gestational diabetes)

  1. The patient must fast for at least 8 to 12 hours before the test.
  2. A fasting blood glucose sample is taken first.
  3. The patient is then given a solution or syrup containing 75 g of glucose to drink.
  4. After drinking the glucose solution, blood samples are taken at 1 hour and 2 hours, the glucose level in each is measured, and the results are then analyzed.

Diagnostic Thresholds for Gestational Diabetes in the 75 g OGTT

TimeAbnormal (consistent with Gestational Diabetes)
Fasting≥ 92 mg/dL
1 hour≥ 180 mg/dL
2 hours≥ 153 mg/dL

In the 75 g OGTT, even one value at or above its threshold is considered sufficient to diagnose Gestational Diabetes.

100 g OGTT (Three-Hour OGTT)

Some centers use the 100 g OGTT to diagnose gestational diabetes.

  1. The patient fasts for 8 to 12 hours.
  2. A fasting blood glucose sample is taken.
  3. The patient is given a solution or syrup containing 100 g of glucose to drink.
  4. After drinking the glucose solution, blood samples are taken at the following times:
    • After 1 hour
    • After 2 hours
    • After 3 hours

The results of these samples are then compared with the recommended diagnostic thresholds, which are given below.

Diagnostic Thresholds for Gestational Diabetes in the 100 g OGTT (Carpenter-Coustan Criteria)

TimeAbnormal Value
Fasting≥ 95 mg/dL
1 hour≥ 180 mg/dL
2 hours≥ 155 mg/dL
3 hours≥ 140 mg/dL

In the 100 g OGTT, at least two out of four abnormal values are generally considered consistent with a diagnosis of Gestational Diabetes.

If two or more results during the 100 g OGTT exceed the recommended thresholds above, the condition is considered diabetes during pregnancy, i.e., Gestational Diabetes.

Although the basic procedure of both tests is similar, the amount of glucose used, the duration of the test, the timing of blood sampling, and the diagnostic thresholds may differ.

What Are the Diagnostic Values of OGTT for Diabetes?

For non-pregnant adults, the following 75 g OGTT values from the World Health Organization (WHO) and the American Diabetes Association (ADA) are generally used.

CategoryFasting Glucose2-Hour Glucose
Normal Glucose ToleranceLess than 100 mg/dLLess than 140 mg/dL
Prediabetes (IFG)100–125 mg/dL–
Prediabetes (IGT)–140–199 mg/dL
Diabetes126 mg/dL or higher200 mg/dL or higher

IFG = Impaired Fasting Glucose; IGT = Impaired Glucose Tolerance. Diabetes is diagnosed when either the fasting value or the 2-hour value meets the threshold.

Why Is OGTT Recommended in Diabetes?

OGTT is recommended to determine whether a person has:

  • Normal glucose tolerance
  • Prediabetes
  • Diabetes

This test specifically evaluates the body's response after a glucose challenge, so it is useful in patients whose fasting blood sugar or HbA1c results are unclear.

When Should OGTT Be Done for Diabetes?

OGTT may be recommended in the following situations:

  • Fasting blood sugar results are borderline or unclear.
  • HbA1c results do not match the symptoms.
  • Prediabetes is suspected.
  • Diabetes is suspected despite normal fasting sugar.
  • A more detailed assessment of glucose metabolism is needed.

Why Is OGTT Important in Gestational Diabetes?

OGTT is considered the most important test for diagnosing gestational diabetes because insulin resistance naturally increases during pregnancy.

In some pregnant women:

  • Fasting blood sugar may be normal.
  • HbA1c may be normal.
  • But sugar after meals may rise abnormally.

By measuring blood sugar after a standard glucose challenge, OGTT reveals such abnormalities.

Without OGTT, many cases of gestational diabetes can go undiagnosed.

Why Do Gynecologists Recommend OGTT During Pregnancy?

Gynecologists recommend OGTT because it is the standard method for diagnosing gestational diabetes. It provides information that cannot be obtained from a fasting blood sugar test alone.

This test determines whether the hormonal changes of pregnancy have affected the body's ability to control blood sugar, which is why it is an important screening and diagnostic test during pregnancy.

When Is OGTT Done for Gestational Diabetes?

In most pregnant women, OGTT screening is generally done between 24 and 28 weeks of pregnancy.

If risk factors are high, the doctor may recommend the test earlier. The final timing is decided by the gynecologist or obstetrician.

Which Factors Can Affect OGTT Results?

1. Inadequate fasting

Eating or drinking anything containing calories before the test can make the results inaccurate.

2. Severe illness

Infections, fever, or other illnesses can temporarily raise blood sugar.

3. Medications

Some medications can affect glucose metabolism, for example:

  • Corticosteroids
  • Thiazide diuretics (used in kidney-related treatment)
  • Some antipsychotic (brain-related) medications
  • Some hormonal treatments

4. Physical activity

Unusual exercise before the test can affect the results.

5. Smoking

Smoking before or during the test can affect glucose tolerance.

6. Mental or physical stress

Stress can raise blood sugar.

7. Vomiting during the test

If the patient cannot keep the glucose solution down, the test may become invalid.

8. Recent dietary changes

A very low-carbohydrate diet can affect test results.

Key Summary

  • OGTT checks how the body handles sugar after a standard glucose drink.
  • It is used to diagnose prediabetes, diabetes, and gestational diabetes.
  • Glucose is given after overnight fasting for the test, and blood samples are taken at set times.
  • In non-pregnant individuals, a 2-hour blood sugar of 200 mg/dL or higher indicates diabetes.
  • In pregnancy (75 g OGTT), gestational diabetes is diagnosed when fasting sugar is 92 mg/dL or higher, 1-hour sugar is 180 mg/dL or higher, or 2-hour sugar is 153 mg/dL or higher.
  • For gestational diabetes screening, OGTT is generally done between 24 and 28 weeks.
  • Gynecologists rely on OGTT because it can reveal sugar metabolism abnormalities that fasting sugar alone may miss.
  • For accurate results, proper fasting, a review of medications, and following the test instructions are essential.
