Khatna (Sunnat) in Boys: Best Age, Types, Healing Time & Aftercare

Khatna (circumcision) is a minor surgery that removes the foreskin. Most Muslim families do it as sunnat, and the safest time is usually the newborn period or first few months, though it can be done at any age with the right method and anaesthesia.

What Is Khatna (Circumcision)?

Khatna (also spelled khatne, khatnay) removes the loose skin covering the tip of the penis. In Pakistan most families choose it for religious and cultural reasons, and some boys need it medically, for example with a tight foreskin.

Why Is Circumcision Done?

  • Religious and cultural: A long-standing practice in Muslim families.
  • Hygiene: The tip is easier to keep clean.
  • Medical: Lowers the risk of urinary tract infection (UTI) in infancy, balanitis (infection of the tip) and phimosis (foreskin too tight to pull back).
  • Other: May lower the risk of some sexually transmitted infections later in life, but safe practices matter more.

Khatna in Islam: Ruling, Sunnat and Timing

  • Fitra: Circumcision is listed among the acts of fitra (natural practice) in authentic hadith narrated in Sahih al-Bukhari and Sahih Muslim, together with trimming the moustache, clipping nails, and removing underarm and pubic hair.
  • Ruling: Scholars differ. In general, the Shafi'i and Hanbali schools regard it as obligatory (wajib), while the Hanafi and Maliki schools regard it as a strongly emphasised sunnat and a sign of Islam (shi'ar). Ask your own scholar which position your family follows.
  • Timing: There is no single fixed age or day. Scholars allow flexibility between early infancy and before puberty. Some prefer the seventh day, but this is not compulsory.
  • Health comes first: Islam's principle of avoiding harm means khatna can be delayed if the child is premature, unwell or has a bleeding problem, and done when the doctor says it is safe.
  • Who should do it: A trained doctor in a clean setting is the safest choice and fully consistent with the Islamic duty to protect the child from harm.
  • Adults and new Muslims: It can be done at any age. Scholars generally excuse a person if it would cause real harm or severe hardship, so a doctor's advice matters.

Methods of Circumcision

Ring (Plastibell)Open (surgical)
Best forNewborns, young babiesOlder babies, children, medical cases
Pain controlLocal numbing medicineUsually general anaesthesia in children
StitchesNoneDissolving stitches
TimeAbout 10–20 minutesAbout 20–40 minutes
After the procedurePlastic ring stays on the tip, falls off in 5–10 daysNeat stitch line; stitches dissolve in 2–3 weeks
HealingAbout 7–10 daysAbout 10–14 days
Main ruleNever pull the ringKeep the stitch line clean and dry
Laser circumcision: The laser is only a cutting tool used instead of a blade. It does not replace a trained surgeon, proper pain control and good aftercare. Ask who performs it, what anaesthesia is used and how long healing takes. Those answers matter more than the tool.

Best Age for Khatna

The best time is usually the newborn period and first few months, once the baby is feeding well and gaining weight. The skin is thin and healing is fast. It is still safe at any age with the right method and anaesthesia.

AgeUsual methodWhat to expect
First 4 weeksRing, local numbingFastest healing; baby must be healthy and feeding well
1–6 monthsRing in most babiesQuick and simple
6 months–2 yearsRing or openOften under general anaesthesia
2–6 yearsOpenExplain simply to your child beforehand
7 years and olderOpenNo cycling, swings or rough play for about 2 weeks

Tests Before Khatna

Doctors commonly advise these tests, especially for older children, when anaesthesia is planned, or when there is a bleeding history.

TestWhy it is done
PT, APTT, INR (clotting tests)Checks how well the blood clots, to prevent excessive bleeding during surgery
CBCChecks haemoglobin (Hb) for anaemia and WBC count for infection
Blood groupFor records and emergencies such as blood transfusion during excessive blood loss
Random blood sugarChecks glucose level, especially if the child is drowsy or unwell
Urine routineDetects a urinary tract infection (UTI) so it can be treated before surgery
Important:
  • Show all test results to the doctor at least 48 hours before surgery.
  • Tell the doctor if there is a family history of a bleeding disorder such as haemophilia.
  • Follow the doctor's instructions strictly.

When Circumcision Should Be Delayed

The surgeon will wait if your son:

  • was born early, is unwell, has a fever or is not feeding properly
  • has jaundice that is still being treated
  • has an unusual penis shape, such as hypospadias (urine opening below the tip), a bend in the penis, or a penis that looks buried in fat. The foreskin may be needed for later repair, so it must not be removed first
  • has a family history of a bleeding disorder such as haemophilia

Tell the doctor about any of these at the first visit, not on the day of surgery.

How the Procedure Is Done

Circumcision procedure steps illustration
Main steps of a circumcision procedure.
  1. Check-up: The doctor examines the penis, weight and general health, and asks about bleeding problems in the family.
  2. Preparation: If anaesthesia is planned, follow the feeding and fasting times exactly.
  3. Pain control: Babies get numbing medicine at the base of the penis. Older children are put fully to sleep by an anaesthetist.
  4. Operation: Ring or open method, usually under 30 minutes.
  5. Recovery: Your son is watched until he is awake, feeding or drinking, and has passed urine. Most boys go home the same day.
Important: Choose a qualified doctor in a clean, sterile setting. Khatna by untrained people with unsterilised tools is a common cause of bleeding, infections such as hepatitis, HIV and sepsis, and injury to the penis.

Before Circumcision

  • Make sure your child is well, with no fever, cold or infection.
  • Follow fasting instructions exactly if anaesthesia will be used.
  • Ask the doctor how pain will be controlled.
  • Older children: explain in simple words what will happen.

Aftercare: What to Do After Khatna

  • Keep the area clean and dry. Gently wash with lukewarm water and pat dry.
  • Apply only the ointment or cream the doctor prescribed.
  • Give pain medicine only as advised, at the correct dose for your child's weight.
  • Use loose, soft clothing or a loose diaper, and change diapers often.
  • Ring method: never pull or cut the ring. It falls off by itself.
  • Open method: do not pick at the stitches. They dissolve on their own.
  • Mild redness, slight swelling and a little yellowish crust are normal during healing.
  • Avoid running, cycling and rough play for 1–2 weeks (2 weeks in older boys).

How Long Does Healing Take?

About 7–10 days with the ring method and 10–14 days with the open method. Stitches may take 2–3 weeks to dissolve fully.

Risks and Complications

Serious problems are uncommon with a trained doctor, but can include bleeding, infection, pain, too much or too little skin removed, and narrowing of the urine opening (meatal stenosis). Rare injuries to the penis are mostly linked to untrained operators.

When to Call the Doctor

  • Bleeding that keeps soaking the diaper or does not stop with gentle pressure
  • Fever, or spreading redness, pus or a foul smell
  • No urine for 8–12 hours, or crying with every urination
  • Ring still in place after 2 weeks, or ring slipping onto the shaft
  • Swelling that keeps increasing instead of settling
  • Baby very sleepy, refusing feeds or unusually irritable
