0-12 Months Baby Cough: Kab Fikar Karein?
A cough in a small baby is scary, especially at night. Most baby coughs are caused by a simple cold and settle on their own, but a baby under 12 months can get worse quickly, so it helps to know what is normal and what is not.
Learn when to worry about a baby's cough, what is safe to do at home, which medicines to avoid, and when to see a doctor (0 to 12 months).
When to Worry About a Baby's Cough
Before treating the cough, undress the baby down to the nappy and watch the chest for one minute. Trouble breathing matters more than the cough itself.
| How Urgent | What You Notice |
|---|---|
| Hospital emergency now | Struggling for each breath, can barely cry, ribs or chest pulling in, grunting, blue or grey lips or face, pauses in breathing, floppy or hard to wake, blood in the cough, heavy drooling, trouble swallowing |
| Doctor the same day | Wheezing or a harsh noise when breathing in, nonstop coughing fits, fast breathing, taking less than half of the usual milk, signs of dehydration (no wet nappy for 8 hours, dry mouth, no tears), fever above 104°F (40°C) at any age |
| Doctor within 24 hours | Any cough in a baby under 6 months (even if mild), ear pulling or ear discharge, fever lasting more than 3 days, fever that returns after a day without it |
| Doctor in clinic hours | Vomiting with coughing 3 or more times, runny nose for more than 14 days, cough lasting more than 3 weeks |
Fast Breathing Rate by Age
Count breaths for a full 60 seconds while the baby is calm or asleep. One rise and fall of the chest is one breath.
| Age | Fast Breathing If Rate Is |
|---|---|
| Under 2 months | 60 breaths/min or more |
| 2 to 11 months | 50 breaths/min or more |
Why Is My Baby Coughing?
A cough is a reflex. It clears mucus and germs from the airway and protects the lungs, so a cough is not always a bad sign. The common causes are listed below.
- Common cold: the most common cause; a virus irritates the airway and mucus drips down the throat
- Dry air: air conditioner, heater, fan or dry winter weather
- Smoke, dust and strong smells: cigarette smoke, loban (incense), chulha or cooking smoke, mosquito coils, winter smog, perfume
- Milk reflux: milk coming back up can trigger a cough, especially when lying flat
- Allergy or asthma: less common under 12 months, but suspect it if the cough keeps returning with wheezing
Less often, a cough is a sign of a serious illness such as bronchiolitis (RSV), pneumonia, croup (barking cough) or whooping cough. These usually come with fast breathing, wheezing, high fever or a baby who looks very unwell.
Baby Cough by Age: 0-3, 3-6 and 6-12 Months
| Age | What to Know | Safe Care |
|---|---|---|
| 0 to 3 months | Any cough should be checked by a doctor early. Fever of 100.4°F (38°C) or more is an emergency. | Feed more often, saline drops and gentle suction, safe sleep. No medicine, not even paracetamol, before the doctor sees the baby. |
| 3 to 6 months | A cold cough is common but can turn into a chest infection quickly. Call a doctor within 24 hours. | Same home care. Paracetamol only for fever or pain, dose by weight. No ibuprofen. |
| 6 to 12 months | Most coughs are colds. See a doctor if it lasts more than 3 weeks or the breathing changes. | Same home care + sips of boiled, cooled water. Paracetamol for fever/pain. Ibuprofen only on doctor's advice. No cough syrup or honey. |
Safe Home Care for a Coughing Baby
There is no safe cough medicine for a baby, so the best treatment is fluids, clearance of nose and moist air.
- Feed more often: breast milk or formula keeps the baby hydrated and thins mucus; if the baby vomits after coughing, give smaller feeds more often
- Saline drops and gentle suction: 1 to 2 drops in each nostril, wait 30 seconds, then use a bulb syringe; do it before feeds and sleep, not more than 3 to 4 times a day
- Cool-mist humidifier: keeps the air moist; clean it daily, keep it out of reach and do not add oils to the water
- Room humidity: keep it around 40% to 50%; too much dampness grows mould
- Bathroom steam (optional): run a hot shower with the door closed and sit with the baby away from the running water for 10 to 15 minutes; stay with the baby the whole time
- Chest Physiotherapy: pat the chest and back with a cupped hand, a little firmer than when burping, to help loosen stubborn mucus
- Hold upright after feeds: 15 to 20 minutes to reduce reflux
- Safe sleep: flat on the back on a firm surface, no pillow and no propping the head up
- Smoke-free home: no smoking, loban or mosquito coils near the baby
- Wash hands and keep sick visitors away
- Baar baar doodh pilayein: maa ka doodh ya formula milk bache ko baar baar pilayein; ulti ho to kam miqdar mein, zyada baar dein
- Bathroom ki bhaap (optional): garam shower chala kar darwaza band karein, bache ke saath 10-15 minute baithein, aur saath rahein
- Doodh ke baad seedha bithaein: 15-20 minute, taake reflux kam ho
- Mehfooz neend: peeth ke bal, sakht sateh par, takiye k baghair sulaein
- Dhuan-free ghar: bache ke paas cigarette, loban aur mosquito coil bilkul nahi jalaien.
- Haath dhoyein: bimar mehmanon ko bache se door rakhein
Keep the Home Free of Cough Triggers
If the cough keeps coming back, check the baby's surroundings.
- No smoking indoors, because smoke stays on clothes and curtains
- Damp-dust and vacuum regularly; a HEPA filter vacuum or air purifier helps if you have one
- Wash bedding in hot water and keep pets out of the baby's sleeping room
- Avoid taking the baby outside on days with heavy smog or dust
- Keep a small "sick kit" ready: saline drops, bulb syringe, thermometer

What NOT to Do
Many well-meaning remedies do more harm than good in a baby. Avoid the following.
- Honey (shahad) in any form before 12 months, because of the risk of infant botulism
- Cough syrups, nazla zukam syrups and decongestant drops, unless a doctor prescribes them after examining the baby
- Vicks, menthol, camphor or eucalyptus rubs on the chest or nose
- Ghutti, herbal arq, kehwa, ajwain or saunf water, which are not proven safe at this age
- Holding the baby over a pot of hot water (bhaap), which can cause burns
- Pillows or propping the head up in the cot
- Over-suctioning the nose (more than 3 to 4 times a day), which makes it swollen and sore
- Antibiotics on your own, since most coughs are viral
- Smoking, loban or mosquito coils near the baby

How Long Does a Baby's Cough Last?
| Cause | How Long It Usually Lasts |
|---|---|
| Common cold cough | Usually 2 to 3 weeks; it can linger after other symptoms are gone |
| Dry air, smoke or dust | Improves within a day or two once the trigger is removed |
| Cough lasting more than 3 weeks | Needs a doctor's check |
It is advisable to see a doctor if: the cough gets worse after day 3 or 4, lasts more than 3 weeks (any cough lasting 8 weeks or more needs full evaluation), comes with night sweats or poor weight gain, or the baby seems to be getting worse even though you are doing everything at home. Trust your instinct, you know your baby best.
